The issue is whether or not people use them, and generally people don't which is true for most Asian countries, specifically those who are more traditional. Good news is that people are far more aware now and things are seemingly getting better

In America yes, insurance can get expensive. There is a government run Medicare and medicade you can get in America but I think there are requirements you have to meet to be eligible. Even then insurance companies in America can deny paying for certain types of procedures.

I have no insurance. I get the default care which is fine because I don't pay out of pocket for it anyway. The only reason you should ever buy insurance is if it's accident work related or if you're planning to do something dangerous like skiing or traveling outside your country.

in japan you can only get permanent residence if you live 10 continuous years there, so this is off the table fir a looooong time
