Aligned with the key Future Focus pillar of EMHS’ Operations Plan, our Home Hospital service achieves its next exciting milestone on Monday 1 July, with the shift to a pilot stage. Designed around an updated model of care based on feedback from the proof-of-concept trial, the pilot introduces new remote monitoring technology to an expanded cohort aged 16 years and older.
The service is underpinned by a multi-disciplinary team comprising medical consultants, specialist nurses, physiotherapists, occupational therapists, social workers and pharmacists, who deliver both virtual and face-to-face care - providing a hospital substitution service specifically tailored to the patient in the comfort of their own home.
This week, we hear from Bige Jones, the first patient admitted to this innovative new service and Home Hospital Clinical Nurse Manager Lorna O’Sullivan.
