Hospital at Home Program

The Hospital at Home program provides qualifying patients the choice to continue receiving comprehensive acute hospital care services in their home instead of completing their recovery in a hospital unit. Daily care is provided by a hospital-based physician, nursing and allied health teams through in-person home care and virtual care with technology that connects the patient with the team back at the hospital. Hospital at Home is different from existing care programs because it provides acute care to patients who require the attention of a hospital team in their home environment.
