Then when you have to leave its more quiet, signifying how when it's all over and the adrenaline is gone, all you're left with is the realization of what you've done.

Canonically, the entire game until Trauma is a dream, dreamt up by Jacket while he was recovering from the gunshot wound in his head. The "hallucinations" are memories of dead Russian mobsters. He did kill all those Russians, and his GF did get killed by Richter, but he dreamt all of it up while he was in a coma.

It's a hallucination of Jacket. The redhead was a squadmate of Jacket's when he was in the military. He died and since Jacket has mental problems, he imagines it.
