This is the story British airways flight 2156. On the 26th of september 2009 a british airways 777 was on the ground at the Robert L Bradshaw airport in St kitts. The plane was to fly from robert L bradshaw to VC bird international airport in Antigua.
The British airways 777 was parked at a 45 degree angle to the terminal. This might seem unusual but it was done so that the plane could use its own power to taxi away from the terminal as no push back tug was available. This was the first time ether pilot operated here and were unfamiliar with the airport. As they were prepping to taxi they pulled up performance data to calculate takeoff parameters. Robert L bradshaw airport has a single runway. Theyd be taking off from runway 07, but they knew that they didnt need the entire runway for the takeoff run. They decided to start their takeoff roll from the point where taxiway alpha joined the runway. This meant that they wouldn't have to backtrack down the runway for a full length departure. This was 100% allowed, their systems confirmed that they'd have more than enough runway even if they started their takeoff roll from the alpha intersection.
After this, they briefed the departure and arrival into VC bird international, the first officer felt that the airport charts at Robert bradshaw international lacked clarity and information. But it was a small airport so theyd be fine.
The first officer took the plane into a right turn and began following a yellow taxiway line. As they reached the intersection, they held short of the runway. During this time the captain completed the before takeoff checklist. They also had to wait bit because the cabin wasnt secure. After that was sorted out they took the 777 onto the runway and lined up.
On this flight, in the cabin was a british airways station engineer and a british airways airport duty manager. They were seated on the right hand side of the plane. From their windows they could see the runway stretching out in front of them. While the plane taxied, they noticed they were on taxiway bravo and figured the plane would make a left turn and taxi down the runway and then line up. However, when the plane did the opposite and took a right turn to line up with the runway they were shocked. They had good reason. The plane was at intersection bravo and intersection alpha, their planned departure point, was all the way back here. The length of runway available at intersection bravo was significantly less than intersection alpha. From intersection alpha youd have about 1915 meters or 6200 feet of runway from intersection bravo youd have about 1200 or 3900 feet of runway.
In the cockpit they sensed that something was off. The captain stated that the runway looked very short and asked the first officer to stand on the brakes while applying takeoff power.
In the cabin, the station engineer and the airport duty manager knew exactly what was wrong. The station engineer ran up to the cabin manager at the front of the plane. He told her that he needed to contact the flight crew immediately as they were in the wrong position. But before anything could be done, the station engineer heard the engines power up. He got to an empty seat in row 4 and buckled in. This plane was going whether he liked it or not.

Not so many I think. These kind of incident happen only if a chain of irregularities and or events take place. And pilots refer incidents like that as they don't wanna die the next flight. There is a chain of witnesses with the professionals around like ATC etc. Yes NO flights are perfects and pilots and controllers make mistakes almost every day. They are human. But the system has a huge amount of margins. The commercial aviation industry is very transparent in countries respecting ICAO standards. Yes bad things under the radar happened in these countries but it's very rare. If the passengers don't understand what's happening at the moment of an incident, they are informed after it most of the time. The investigations and their results are public. There are always specialized journalists to report it to the wide audience. Ok a bird hitting the flightdeck window is not making the headlines and the passengers don't know. But for example, a mechanical issue leading to no critical situation, they are aware of. The last scandal showing that it cab happen is the 737 MAX issue. But nor the Airlines, nor the crews, nor the maintenance knew the problem. They trusted the FAA. FAA was corrupted as Boeing was... It's not the same story of having an incident with non dysfonctional designed airplane. Some incidents are not close to disaster. Some as this one, are. From a former private pilot having be very close to Airlines crew and operations. The exigence in this field is so enormous that the worst thing is that it is often too much for the personnal. You can loose a job you worked hard for years so quickly because one day, your performance has declined a tiny bit. It's very hard, believe me. It is to garantee a level of safety even if machines get failures and humans make mistakes.

There was another incident that I saw before my eyes but never heard any of it in the news, local or aviation news. It was a late night in 2007, a BA jumbo jet 747 was about to touch down in Boston Logan airport when the crew made a final right turn from its northern approach, heading towards the town of Everett instead of the actual runway. Just like the passenger who wanted to warn the flight deck of the error, I felt so helpless to see a giant object with lights aiming at high tower buildings; however, they luckily realized their mistake and veered back onto the runway using its lights as a reference guide. The next evening I went out to see that the buildings’ lights had been dimmed. It’s an industrial area that had been using really bright lights at the top of four towers, but this was the first time I had ever seen a plane mistaking them for airport lights.

Also never reported was the United Airlines 747 that lined up on the A4 instead of 27R at Heathrow. The pilot got to 50 feet before he remembered that no runway on the planet is lit with yellow sodium lights, nor is it commonplace for there to be cars all over the runway. United tried to deny the incident until CAA (Civil Aviation Authority) threatened to ban them from UK airspace for failing to respond to a safety enquiry.

That same scenario you mentioned also played out on a small island or someplace south of Mexico I think. It was featured with on this channel or one like it. Maybe it was the same incident? Not sure but it sounds exactly the same.

Yeh - that almost certainly would have been a key failing if there had been an accident. Looking at the taxiway layout, the controller should have been curious as soon as they missed that link they needed to join B taxiway.

@@rrice1705 It wouldn't be a good thing, but these aircrafts are designed to allow for climb out speed on one engine. It might not set any speed records, but it will be enough to get some altitude for safety and then come around to land. The only thing that would have fucked that up would be that the shorter runway distance may have resulted in a too low beginning vertical airspeed. Those GE behemoth engines are the most powerful in the world. Just the size of these engines are awe inspiring!! I'm thinking the engine power should get them through though, ✈✈

I was an Senior FO on this fleet at the time and flew into St Kitts a week later. I took lots of pictures for the company to check taxiway marking and signage. I can understand why it happened. The B777 is on a island hop flight to Antigua before refuelling and getting fully loaded with passengers to London Gatwick. Due to very light weights, thrust required is much less than max de-rated thrust allowed so performance is greatly enhance for an ‘A’ intersection. Thus they got away with it from ‘B’. The issue would have been a high speed rejected takeoff (no enough runway to stop). Capt A320

6:02 Actually, it's okay to use *either* "the crew go" or "the crew goes"-it just depends where you're from as to which is considered correct. North Americans see it as one entity, and so "the crew goes" is correct. Brits, and I think some other English-speaking countries, look at collectives as plural, like if you'd said "crewmembers", and therefore, "the crew(members) go".

Sometimes for fun i like to make up sentences with collective nouns following the British model, for example: "His hair is falling out." versus "His hair are falling out."

@ RobinHood : You are correct, sir. In my opinion, the British way is incorrect, because a collective noun, while representing multiple things, is itself a single thing. A rock group is a single thing; a sports team is a single thing. So it should be "Led Zeppelin was a popular group" or "Manchester United is a successful team", etc. ____________________ (For what it's worth, both of my parents were British, as are all of my extended family members! 🇬🇧 )

All directly involved suffered from "Confirmation Bias", meaning that even though things didn't look right, they still believed all was good. Tower also believed the pilots knew what they were doing, and the flight deck likewise believed that Tower would stop or redirect them if something wasn't right -- all based on their collective past experiences. When no resistance or apparent confusion was encountered, they further affirmed in their minds things were good, when in fact, they were quite not. I can only imagine they all had that sickly dark sense of dread once the plane passed the point of no return on the ground, once they realized their grave error. The pilot in command most likely realized the grave situation they were in, and then acted quickly to get the plane in the air and then level to accelerate, in order to prevent the unconscionable. Miracles do happen, and this was *definitely* one. I'm also elated that corrective measures were put in place at the airport BEFORE a tragic accident happened. um... Well done?!? Indeed.

What nearly killed them. Not questioning why there was another taxiway beyond what they thought was alpha. What saved them. Questioning if they had enough runway and going full power before roll. Definitely on the airport management for not marking properly, but a little more questioning from the crew might have prevented this issue.

Definitely no denying the fact that this *could* have been a lot worse. The airport seems like it was woefully mismanaged at the time. I hope it’s better now - and that we never see a crash there!

This has been a problem before. Yet nobody put up the proper signage - or even a sign “you are near the *middle* of runway 07: 2213 ft 1931 ft” (Numbers obviously made up) And the ATC should have said something “I see you, you at intersection B, are you sure 1931 ft is long enough for you? Just making sure …”

Just saying "Runway 07 at intersection Bravo, cleared for takeoff" would have been a clue, sounds like nobody specified which intersection they were planning or they were at.

@@chrisschack9716 : I notice that the pilot said "we are good to go from position Alpha" when in fact they were at position Bravo, which should've put the controller on notice that the airplane was in the wrong place, and that the pilot wasn't aware of that.

I presume it came in from London, and than made the 100 km hop to Antigua on the remaining bit of fuel. Would they have been fully refueled for the return flight to London they never would have made it. What makes me wonder, is that the gps navigation board did not show them on a moving map where they were taxing. And many airliners have this system on board where a voice says "approaching runway 07, 1500 meters remaining". I think it is called the RAAS system. It even says "caution short runway" when you attempt to take of from an intersection that would result a too short runway length remaining from that intersection. And the missing taxiway signs and the twr atc failed to notice to the pilots that they are on the wrong intersection. Fortunately this time all ended up ok.

The controllers were partially to blame. But of course the ultimate blame falls on the pilots. But I have made this same mistake at this airport, so give them a break, the old charts were terrible.

Not a pilot but have been following Aircraft investigation since the 80's and know its a chain of events in the aviation industry that leads to disaster. If the chain is broken nobody hears about it. Mr. Mike Stone love your nobility, but understand sometimes pilots are set up to fail due to bad info. Stay safe up there..

At least one party should (NOT could) have requested clarification about the aircraft's position. No professional pilot or controller would be upset by somebody being prudent.

I would try to go to A and then taxi to BEGINNING of runway...always. More is better in this case.

I don’t understand why you wouldn’t always use the whole length of the runway. It’s an extra margin of safety if anything goes wrong.

david chappelle, if you are in a GA plane and there are heavy aircraft ahead it might be safer taking of at an intersection because of wake turbulence. But in something like a 777 I have no clue why they did that.

So they didn’t have to attempt a 180 degree turn at the end. And in another scenario if you have a plane on approach you can just depart from the intersection and clear the runway before the landing aircraft. Otherwise you’ll have to sit and wait whilst that plane lands

I agree. Even in the little tiny plane I fly. If I start at the beginning of the keys, I'm airborne before the end of the keys. I still go right to the end to start my takeoff. If I have an engine problem I can and have rejected the takeoff 100 ft in the air and landed back on the runway straight ahead. Nothing so useless as runway behind you. I don't *need* all that runway. I could take of across most runways. But if something isn't right it's great to have it.

@@gasdive your plane takes off at much slower speeds than a commercial airliner. Even if an A320 tried to do the same thing it will overrun and risk injury or death. That why pilots calculate the V1 speeds to know when they must continue at any intersection

It also seems omitting the taxiway briefing is not an one off occurrence either. Especially when pilots are not familiar with the airport, regardless its size.

You've posted SO MANY videos where "fear of speaking up" has caused close calls or full accidents. Why is this such a fear? If I am the ATC, you bet I'll be questioning people's actions, my job IS TO CONTROL TRAFFIC.

Didn't see any reference to a control tower - or request for ANY intersection takeoff - so I have to assume no tower on the field. ATC controls airspace, AFTER you get there...

Yes. If you ever get a chance to listen to JFK or LAX ATC to Pilot conversations (you can on liveatc website), you'll find ATCs double, triple checking and sometimes even sternly coaching the pilots on numerous occasions. It's sad that it is not the case here.

@@yoopernow : If you look at the map of the airport (for example, at 0:54) you can see that it has a tower, located roughly at the midpoint of the runway, on the opposite side of the runway from the terminal.

And why planes aren't fitted with ground sat-nav graphical navigation, like a car, where you can see where and how positioned you are on the airport, and possibly even have satellite image layer. Anyway... blame also on BA for letting their aircrafts land on a such dilapidated airport where even marking and signs on the ground are missing. And yes the behaviour of the pilots isn't healthy at all. They should have confirmed their positioning and should have gone the full route when they felt they weren't where expected in an already "narrow" choice situation.

I swedish scientist and inventor (Håkan Lans) who was involved in developing a GPS systems for aircrafts (STDMA) during the 80s and 90s was under heavy pressure from the US radar industry (I even think he received subtile death threats). The airline industry was alarmingly conservative and slow in adapting GPS technique. A simple smartphone would have prevented this almost disaster.

Edit 2: Man, with those taxiway charts I can hardly blame them. And no signage? The fact that they only fixed it after there was almost a major incident is concerning.

@@prycenewberg3976 Both my grandfathers moved to St. Louis to work on airplane design after serving in the military/WWII and worked for McDonnell Douglas / what would be Boeing. Neither one would have let such a horrible design flaw slip past them as happened with the 737 MAXes and would be horrified; at least they did not live to see it. The company just isn't what it used to be.

Don't worry about ICAO rules regarding signage; don't worry about taxiway lines; don't worry about charts that lack sufficient context; don't worry about unexpected actions (not back-taxiing from B); don't worry about radio communications indicating the wrong taxiway. Nice! There was a similar incident in SKBO with an AF A340-300. The type was changed after that date and now its only 787-9. ! You should do that one too!

This is the perfect example of how irregularities seeming each of them insignificant can lead to disaster when coming all together. No correct hold short panels is very dangereous. At least if there were NOTAMs mentionning this at the time, it could have give the crew and the trainee controler a trigger to understand something was wrong. I think even with a taxi briefing, it could took place. When things are as confusing as this, any one can fail to identify the threats. This also show how unfamiliar places can challenge a crew. I understand the reaction of the trainee controler even if it's clear that he made a mistake not considering the answer naming taxiway B. But I don't understand his supervisor. He was doubting but did not intervene. At least the trainee questionned himself and refered to another one to verify. The supervisor did not ! What's that ? In aviation communication, when there is a doubt, you must clarify ! The supervisor assuming that the crew knew what they are doing while he was shocked that the 777 departed from there is a bit shocking to me. I mean, he was troubled. What is the cost of asking ? The cost of not asking ? It ended well. It could have been catastrophic. I can't even imagine the feeling of the one understanding what was happening in the plane...

Plane a bit heavier, wind going against you at the last minute, sudden technical issue. This was just an all around lucky escape, it could just as easily have ended with the plane in pieces in the ocean.

Wow, it was good the 777 had enough thrust to propel it down the runway fast enough for the short take-off roll that was only about 3,900 feet, it was also good that the airport charts were updated to provide flight crews with clear information about the airport to prevent similar incidents from occurring again.

A lot has changed since that incident my friend :) !!! I run a Aviation spotting channel from St. Kitts .....British Airways flights to St, Kitts are shared with the island of Antigua just 15 minutes away and so the 777-200 would in most cases be on the lighter side as they would refuel and take on more people in Antigua....i got countless of footage of the GE-90' in action !!!!

That was a great CRM Video thanks. Luckily no engine failure before VR!. No dig at BA and I am a narrow body Airbus Capt who teaches CRM occasionally. One of the Companies SOP and in the OM I worked for in the past was NO Intersection takeoffs at Unfamiliar Airports. I noted your vid says both Pilots had not flown here before right. Put that with dodgy airport charts, new bays etc and no Signage showing hold B for runway and a sign showing A this way etc this IMHO is a classic swiss cheese scenario. I am sure lessons were learnt here as well IMHO. I wont waffle on but Thanks again M.A.C.I.

Not necessarily. Reduced thrust calculations take into account density altitude, wind strength and a whole host of other factors. Basically, for the same weight, and runway condition, one would tend to rotate at pretty much the same point along the runway, no matter the temperature - the thrust produced from the engines being reduced/increased accordingly. (We do this to save engine wear, and hopefully reduce the chances of an engine failure). Light winds, or indeed increased weight, would (if not TOPL limited), see you rotate even earlier, as it’s your performance in an RTO that becomes the limiting factor. If one becomes TOPL limited due weight or environmental factors, then they’d be using the full runway length, and this whole scenario wouldn’t have played out. Oh, and as far as using the full runway length vs an intersection (I know you didn’t comment, but for others reading) - it’s quite common, particularly if using the full length would involve a 180 turn to utilise it. The most tyre wear is incurred during taxi, and tight turns, particularly 180s are very bad for the tyres - so again, we try to avoid them wherever possible. There’s quite a difference between operating a ‘jet’ engine and light aircraft wrt power set at take off. Had this been a GA aircraft that only just had the performance to make it from A, then yes, they would have been in serious trouble from B (the equivalent of being TOPL limited as mentioned above).

Even if the engines started spoiling up for t/o power, the engineer who alerted the cabin manager should have insisted the cockpit be called as a matter of an emergency situation on the phones located by each crew seat. I don’t understand why he would have just sat down and thought ‘ah well I’ll just leave it then, see what happens…’ seems a little odd

“The crew go” is a correct alternative to “the crew goes,” depending on which side of the Atlantic you’re on. The British see group nouns such as “crew” as plural. (“The government are.”) To me it’s a bit overthought, but seems to have become traditional. Correct me if I’m wrong.
