I've just been sedated today for the first time. The wooziness hit me very quickly, and I must have blacked out pretty quick. All I remember was a brief moment of going "ahhh!" which was probably as they administered the anaesthetic injection, and I didn't see anything. Next thing I knew, I was being wheeled into the recovery room. I was completely unaware of the procedure, and I remember trying to ask "is it done already?", but I could barely speak. They call it 'conscious sedation', but I think that might be from their perspective during the treatment because, to me, it felt like being switched off and experiencing a time jump. Like many of the others here, I am very anxious about medical procedures, and I already felt like I was going to pass out just from the cannula going into my hand and the other prep they were doing. I guess the best thing about the experience is being unaware of the procedure. The very best of luck to anybody else who's going for one of these soon.
@beardedbros72155 ай бұрын
It’s called “conscious sedation” because you can still respond somewhat to touch and some sounds. Otherwise, it’s similar to being asleep. I was going to have it done earlier today. But I felt uncomfortable and too anxious.

I've never had surgery before, and the thought of it makes me want to throw up in fear. I did the one thing you shouldn't do, and that's to google it. This time it has actually helped me so much! Seeing the patient's reaction and relaxed state made me feel more secure about my procedure coming up here in about a month. I have to get 9 teeth pulled in total. 4 wisdom, 4 molars, and the bottom right molar closest to the wisdom tooth. the 4 molars is due to over crowding in my mouth (making room for Invisalign), back molar is because it's just too far gone, and the wisdoms... Well because they're wisdom teeth lol. They (surgery team) said the whole thing should take about an hour, and that I'll be put on the twilight sedation. They guaranteed me falling asleep, but I don't know how my body will react to it. I'm still deeply terrified, but this video is actually a godsend. My anxiety is so bad, that I actually calculated how long each tooth extraction will take depending on the complexity of the tooth. My estimation matched up with the teams', which puts me of sound mind.

I had the first part of an implant recently done (tooth removal and ridge preservation). They gave me one hydrocodone and one triazolam about 30 minutes before starting. I felt the effects a little and they asked if I was okay and wanted to be more sedated, so I took another triazolam. Once they started, the only thing I remembered was getting the local shots and the doctor saying I would feel a little pressure while they removed the tooth. I don't remember getting up to leave, or the ride home, or going to bed. I slept about 8 hours, ate a little something, and slept another 8 hours or so. At least I had absolutely no pain afterwards. I didn't even have to take ibuprofen. I think when I go back for the next part of the procedure, I'll just accept one hydrocodone and one triazolam.

For those of us who suffer from severe vasovagal syncope, allergies to epinephrine, and severe dental and needle phobia, I'd like to know what the protocol is for sedating someone who's already in syncope, or who goes in to syncope while sedated. My nightmare scenario is having my blood pressure fall through the floor, becoming dizzy, getting pins and needles, the cold sweats, and going 90% of the way to passing out before then being sedated or during sedation, and being aware *in that moment* of the absolute nightmarish shit hole than I'm in. I was being traumatized but I couldn't do anything but writhe in agony with the necessity of the tooth being extracted. I nearly leapt out of the chair and could hardly breathe, possibly had a stroke/heart attack during the procedure and now have severe anxiety, ptsd from the ordeal. Honestly it's better not being sedated. The local anesthetic takes care of the pain so that's no issue. Foregoing sedation just means you're conscious and of sound clear mind throughout the procedure. Keep in mind that the active ingredient used in this sedation is part of the same cocktail of drugs used in lethal injections for execution of convicts on death row. Even though I always thoroughly vet the background and credentials of any medical professional involved in my treatment, I personally wouldn't allow anybody to inject midazolam or any other benzo into me. Especially not for a non-essential procedure like wisdom tooth removal.

the gas helps but ime it was not enough for the pain and anxiety. i would strongly reccommend the iv sedation with midazolam if possible. never tried propofol as my country only use it for general anaesthesia - i know it is shorter acting but presumably they should have kept it topped up. the other thing is propofol is a deep sedation so you will probably be less able to communicate. with midazolam you are awake & will be able to tell the dentist if it hurts.

👋 Anyone reading the comments about to have their surgery … let me explain my experience. ( I got them out 5 hours ago ) I WAS A NERVOUS WRECK before hand watching every video possible reading all experiences on reddit and comments on these videos from the good ones to the bad ones. I’ve been nervous since they told me last year they were coming in. It was a peice of CAKE for the people like me who have severe anxiety about medical stuff you will be just fine! I wasn’t worried about the recovery more so the surgery itself, and a little stressed about the IV ( i was put to sleep with IV sedation ) the IV was NOTHING me and the assistant were giggling and I was asking if I should watch him put it in or look away and he promised me it was nothing and maybe id enjoy watching it go in.. I looked away 😭 but it barley felt like anything, like a poke a tiny little prick. they put the thing on to check my blood pressure every 5 minutes and the thing on ur thumb along with some like patches they put on my lower side and chest. Sat there for about 30 minutes with the IV in my arm until the doctor was ready. He came in asked how I was lowered the chair down and put this thing on my neck to monitor my breathing I think he said,, and then he released THE GOOD ( the fluids into the IV ) it was a crazy experience.. like i was out but wasn’t ? all i last remember is seeing is the light and then within seconds it was done. I was so stressed for NOTHING. so glad I went with IV sedation because I didn’t feel a darn thing! 😂 and the loopyness afterwards ain’t all that bad either. I would definitely do it over again and take away all the stress from before because it was nothing.

Can I ask for advice please? I need to get out a wisdom tooth and the tooth in front due to the sideways rear wisdom tooth damaging the tooth in front over the years and now has decay into the root of that tooth. I suffer from severe anxiety so you would think conscious sedation would be perfect but my anxiety is such I fear sedation of any type and this type even more as I would not be in full control of my faculties. Too much info I guess but I was drugged and abused as a kid which is where my anxiety came about and I have even put off getting a suspect tumor taken out a long time ago due to this fear of sedation but lucky it turned out to be benign. I fear I might talk about the abuse under this type of sedation as well given its so strongly embedded into my fears and anxiety that it's likely to be the thing on my mind going into this type of procedure and the loss of higher conscious would leave me vulnerable to those fears coming out when I talk without meaning too. I don't know if that makes sense to you but I hope it does. While they don't want to do the work without sedation can it be done if I talk to them why I don't want it? Sorry to ask such a question on youtube but my fear is getting to me over it. Thanks.

it's late... for future commenters (hopefully your teeth are ok now) i am terrified of the dentist due to sexual abuse when i was younger. as soon as the drug goes in your anxiety will go away. from crying and shaking with fear you will be relaxed and happy. the idea is scary but the experience is so good. now objectively: if your dentist is recommending sedation it is probably for a reason, either due to your past anxiety or the procedure. but you have every right to ask them to do it without sedation. that's the ideal as it's the least risky for side effects etc. the only reason they could still not want to do it without sedation is if you are unable to keep still & let them do the treatment normally. then you will have a hard decision if you are against sedation... i have avoided dentists for years & it isn't fun last thing is i am 99% sure your dentists will not be shocked or offended if you express your worries. it is a scary thing to be sedated! it would be good to call them.way before & discuss everything. i told the dentist before anything happened very frankly that the reason i was afraid was bc of sexual abuse & she couldn't have been more.understanding. it's a very common thing.

It's hard to say without knowing your medical history however, as a surgeon it is always best to practice safe and conservative medicine. When I train other dentists I teach them the importance of recognizing their strengths and weaknesses and acknowledging that it is "ok" to refer out. The doctor may not be untruth to you but rather, practicing in a manner that is more comfortable to his training and abilities. If you have concerns, I always recommend a second opinion because not all doctors are trained equally nor do we all understand the same concepts equally.

When a patient is under the influence of sedation medications I always refrain from using any type of topical anesthetic that provides 'some' numbness to the mucosa of the mouth. I do this because the topical anesthetics can be swallowed or aspirated and this could affect the patient's airway. And I never want to compromise the airway by adding an irritant which can cause coughing, excess saliva, and possible throat construction. Also, it should be noted that the part of the injection that she is 'feeling' is not the needle passing through the external oral mucosa, its when the needle passes through the masseter muscle. No topical anesthetic administered prior to that injection will provide numbness to that deeper muscle regardless of the use of sedation or not.

I have been told by many dentists it is rather complex, so part of me is hoping for the hospital. But I am open to sedation. For my mesial it’s under gum - will I feel that being done at all? The narcotics are definitely the cause of nausea and vomiting but rarely happens during the procedure. The use of the word 'common' is less dependent on the drug, but more the result of the person administering the drug. Conscious means that the patient is conscious during the appointment. They are able to communicate and understand our instructions. The patient has the same personality and reactions however, the sedatives will create relaxation and amnesia. This means that over time as the medication is eliminated from the body, many of the memories of the procedure are erased as well. It seems very obscure how it all works, but as long as the patient is comfortable and relaxed we know that the medications are doing their job. If the patient was undergoing general anesthesia they would not be talking or responding to anyone. They also will need some additional management of their airway so that oxygen can get in and out of their lungs as some patients will stop breathing on their own while under general anesthetics. Conscious sedation allows the patient to breathe on their own and also protect their airway. This is why it's a great level of sedation for the general dental practice.

From the sink... which is dirty... really dirty for some people... we don't ask them to use sterile water. Why? Because the overall strength of the human immune system should be able to withstand those bacteria they encounter daily. The same is true of sedative drugs. Professionally speaking, I have never read any specific research or literature pointing out any immunosuppressant effects of midazolam that dentists should be concerned about. If you are concerned about post operative infection and healing I would strongly suggest using a product called SOCKIT! Gloves don't equal sterile field. Gloves only protect the provider from fluids on the skin. That's why it's called PPE (PERSONAL protective equipment), that doesn't protect the patient. The needle is sterile and kept so until it goes into the skin. Alcohol wiping isn't even sterile, it just cleans the area. And that's why we have immune systems. This wouldn't cause a blip in the patients health. Hence we don't die from blood draws or other skin breaks. And FYI, most dental offices have better aseptic and infection control techniques than most ER and physicians offices because we deal with far more infectious materials than a simple IV start.

Is conscious sedation the same as twilight sleep? Just use different names sometimes?
@ginal.salatinodmdfagd66405 жыл бұрын
The textbook answer is no, they aren't the same thing. However, many dentists, patients, and other practitioners will use the terms synonymous even though they are technically different levels. Conscious Sedation is also known as Moderate Sedation. Twilight Sleep is also known as Deep Sedation. Because sedation levels are a continuum, it's not uncommon for patients to drift into Deep Sedation if we are suppose to be working in Conscious Sedation. That's not the goal, but it easily happens and we are prepared to treat the patient accordingly. In most cases when patients go into Twilight or Deep sedation we simply continue to work and make sure the airway is open and the oxygen is easily flowing. Most patients will recovery back into Conscious Sedation after a period of time and we have no need to do any specific treatments. In both levels of sedation patients can have amnesia although it's more guaranteed to happen with Deep Sedation. I hope that helps answer the question without adding further confusion.

Those are great questions. Moderate sedation is a level of sedation where the patient is still conscious and has all of their normal feelings and sensations. The sedative helps patients become very relaxed, and creates amnesia (memory loss). General anesthesia is the only form of sedation that takes patients deep enough so that they do not respond to the pain. It also creates amnesia. General anesthesia is usually reserved for hospitals and out-patient facilities. Although the patient seems normal, she is very much sedated and has no memory of these procedures.

So in other words... you tortured her... but the drugs helped her forget it even happened. Lovely.

It's not torture if patients understand that there is always some sort of 'sensation' attached with the procedure. Torture is when you restrain a human and continue to deliver painful procedures with the intent to do harm. The patients are aware that they will FEEL things during a sedation appointment. That's part of being human and conscious. In these procedures we adress the discomfort, give additional anesthetic or anesthesia, and continue. The patient wilfully sits through the procedure, thus it is not torture. Sedation at this level does not remove the patient's ability to move away or stop us if they are in extreme pain.
