As usual … a good physical therapist is worth 10 orthopedists. An orthopedic Doctor’s only answer for joint issues is pain meds or surgeries. Thanks for the knee exercises … I’m bone on bone in both knees, and have been told I need replacements. Your exercises have helped me tremendously in the last couple of days. Thank you, Dr. David. I did your exercises many months ago, after being told, that if I needed a third injection in my knee, whch was diagnosed as the beginning of a miniscus tear, I would need surgery. The pain was acute, in the inside of the side of each knee, the left being worse. I had hyperextended both doing an exercise. I turned 78, in June, and since then, my puppy mill rescue was diagnosed with kidney failure, and my husband has dementia...SO...I was changing doggie diapers to a blind, and deaf dog, and having to take over the bills, and finances, plus several other learning curve items. SO...I truly am grateful my knees are awesome. We had to put the dog down, after he suffered a bout of pain, and stopped eating. Add depression to the menu. I am a strong person, and I am fighting, and NOT having to deal with that pain is a true blessing. You are truly an awesome person for sharing your knowledge, and experience just to give others freedom from really acute pain. I just purchased one of those mini chain saws, and I am sawing limbs with ease. So glad I am able to do things many younger people find difficult. People need to know that they don't have to give up. Movement keeps us healthy all over, and sitting makes many pains worse.

Hey @rndst6, we are so glad to hear you resonate with our message! 🙌🏼 There are many reasons why overemphasizing quad work can worsen knee pain symptoms. The key is in finding the right balance, but it's very important to become obsessed with having more glute strength to effectively offload your knee!

Hello David, coming off meniscus and chrondroplasty knee surgery which is pretty angry still. I am off crutches, using a cane but still having pain. My PT is doing all the things you say not to do. It's depressing. I find it hard to tighten my glutes standing.. I can do it on the table.. your ideas are unique and make sense.. Is there a place we can print your ideas? Thanks so much!

I had miniscus surgery in 2003, and then told i needed knee replacement by an orthopedist...I have been trying stretching and strengthening exercises since then, and told 2 more times by 2 other doctors i needed knee replacement, but today(2023) I am walking better than anytime in last 20 years, and i continue to stretch my knees and to do low impact exercises like bicycling..I do not know exactly what is happening except that I am improving all the time, and I follow your videos, and really wish more doctors would give people more options than just knee replacement..thanks again...

Hey Doc, i'm a fairly high-level tennis player in my 40s (competed all my life throughout school), and I am pretty hard on my knees (as you could see from my videos). My ortho just told me this week that my MRI says that if I keep going like this, i'll likely have to get a knee replacement within 20 years. He diagnosed me with Osteoarthritis. Finding your channel has given me hope that I won't have to stop doing what I love... been doing your exercises, esp strengthening the glutes, and massage gunning the quads. I used to do the quad stretches that you talked about in this video... now I see how ignorant that was lol. Thanks for all your help, Doc.

Swinging the lower leg for 45 minutes a day can work wonders in growing new cartilage which orthopedic surgeons have said couldn’t work

Hey Dr D I've been doing ur knee program now for 3 months and having great results. I do the exercises every single day. Going up and down stairs is so much easier now again. I'm also doing ur shoulder exercises - results r a little slower. Please please do more videos for shoulders. Thank u so much for all ur hard work and time. No one else teaches this stuff !

I had a really bad fall nearly two years ago. I had scans ,and pain killers etc. nothing has really helped , BUT a a few weeks ago I saw a woman on you tube with all the same problems I had , knee giving way ,especially on the stairs , and if I turned suddenly, even when driving moving foot from accelerator,to break . But she went to chiropractors, and I watched what he did , and showed my daughter , as she is a massage therapist I got her to do this lymph drainage, and in just two weeks , this has helped much more than anything else . She is spending an hour on my legs about 2/3 hours a week. It’s incredible the difference . REALLY …

Hi I’m from Winnipeg! I suffer with bone on bone knee pain and I haven’t seen a surgeon yet, not for another 3-6 months and don’t expect surgery for 12 months afterwards! If I can find relief without surgery, I’d be thrilled

I remember a bang to my knee that took weeks...months that took to heal completely...it was bad and paintul... Luckily today it has not come back to be painful as I get old...crossing my fingers and taking a joint supplement for decades..hope it helps Thanks for the talk

I'm not sure where I got the idea to strengthen my quads, but I kept trying to do that. And stretching them as well. The physical therapist that I went to did seem to stress the quads over all other groups (ouch). Imagine my surprise when isometric exercise that focused on the entire leg musculature ended my knee pain for two days. I'm an old unit and I understand that I need to keep good care of myself nutritionally, staying hydrated and moving within my limits and avoiding injury, but I now happily put the effort into those isometrics that engage the entirety of the feet, calves, quads, and also include the hips and the lower abdomen. It's helping me to walk more easily and safely. Who'd a thunk you could sit in a chair and get stronger? Thank you for sharing with so many people and helping us all learn more.

Been 4 years - i retired because of torn meniscus and a couple spots worn through the cartilage . Haven’t really done anything , other than i stopped walking too much . kind of surprised that I haven’t had any pain issues with the knee . Would like to get another MRI , but haven’t found a doctor who isn’t a pill pusher . ** I’d like to see a video of someone who actually healed or even slightly improved a bad knee

From Caroline in Zimbabwe 8th April 2023 - 72 years old - hyper flexing injury 10 months ago - MRI showed torn cartilage - was able to play some tennis - but unable to get full knee extension - decided to have arthroscopy 3 weeks ago. What you are showing on this video - MAKES SO MUCH COMMON SENSE - I feel I can improve my cartilage somewhat now - (PS I am a trained physio but did mainly hospital work and rehab not much sports stuff) - thank you EL PASO

I've had a mensicus tissue tear 4 months ago and still doing physiotherapy on my own at home. No pain but a little bit of stiffness when I sit for a while. I really appreciate the video reg walking and exercise to improve the quads. How long should I continue physio..? I'm 67... before my injury I used to walk 8 kms in the morning and cycle 5 kms in the evening. I think this caused my tear due to my age.. ?? Otherwise I'm fit and weigh 52 kgs. Do all my house work, drive etc as it's my left Knee and and no clutch to operate. How long do I need to wear a knee sleeve or brace..? Thank you Got alot of information regarding my knee.

I'm not a professional, but I've been playing soccer my entire life and I train hard, I do cardio, HIIT and I strengthen my leg muscles. I always thought having stronger quads meant less chances of getting injured, but after watching this video I guess I was wrong. After 10 years, I've been experiencing cartilage wear issues and bone spur pain again, I was halfway through my recovery and thought it would be good to strengthen my quads again and I made a huge mistake. Now I'm regretting it, I just don't know how often it is safe to do quad strengthening, how much weight, reps, etc... But letting my muscles and strength go away forever is also a huge no no... Moderation! I just need guidance...

bro it's ok to strengthen the quads, he mentions in other videos that the problem is that most people have very weak glutes, once you strengthen and build up your glutes to work properly it is then ok to work on your quads in conjunction with the glutes, as long as the quads aren't so strong that they are taking over for the glutes...you basically want strong BALANCED leg muscles, all of them, calves, tibialis anterior, quads, hammies, glutes....the problem is most these old outdated PT's ONLY tell you about working on your quads, they avoid everything else and create more IMBALANCE

Hello Seema! Swimming can be helpful as long as you're not over using your quad muscles. If you have swelling right now, you have got to get it down before you go into aggressive exercise. I couldn't bend or straighten my knee at all for 2 months. While it was painful to do, I was succesfully able to restore at least 95%! I can fully straighten and bend it now. You have to get creative & push past the discomfort, once the swelling comes down a bit!

I really enjoy your videos, very helpful. I had surgery to repair a meniscus tear in my left knee 7 months ago. I went to PT for 2 1/2 months post op; their therapy included strenghtening the muscles around the knee (including the quads), doing lunges, squats, and weight bearing exercises, exactly what you don't recommend! I have arthritus in my knee, and walk with a slight limp. My range of motion is about 80%, there is slight discomfort when I walk, kinda tight. My DO took another xray and said I have bone on bone and reccomended knee replacement. I'm at a crossroads. I'm very active, golf, tennis and surf, and still do them but at a decreased intensity. Would a knee replacement get me close to full mobility, or can exercises and streches improve my bone on bone and daily arthritus discomfort?

Repair ? Or did they just remove torn cartilage ? I have torn meniscus and a couple holes in the cartilage from walking with a bad ankle . When the knee pain started doctors wanted me to take pills and get the arthroscopic surgery . I know people who have done that and it was just a couple years and they had to get replacement surgery . I retired and stopped walking . I can do several hours of yard work and my ankle starts hurting . I am surprised my knee is ok for almost 5 years without PT . I realize injuries are different , mine is the old age worn out type

Hi Doc , really loving the content . long story short few years ago had ACL and meniscus reconstruction , last month while playing basketball I have re injured that knee, gave way when Ianded on it, had an MRI the graft is still in place however I have damaged the cartilage , now grinding / popping when I'm straightening or walking . What would you advise so my knee doesn't give way again while playing basketball and how long until I can stand playing again

Hey Carl, sorry to hear that you re-injured your knee :-( You definitely need to let that knee heal which could mean you need to back off on sports for a bit...really as long as it takes for you to feel better. Cartilage is the slowest healing part of the body. It could take 3-6 or even 6-12 months depending on how bad the injury is. You definitely need to work on an muscle imbalance you might have. For basketball, you must gain A LOT of strength in the glutes to tolerate all the jumping and cutting. It's really easy to get the quads over-dominant while playing basketball. Thanks as always for your great advice. I had a car accident and it created a 4-5 inch hematoma on my shin below my knees. My left knee turned outvok, but the nerves in my right shin really protested all the rubbing at PT. I still can barely touch it, today, 3 years later. That said, my right knee is just acting up. Plenty of space, but I can't stand long on it, can't turn it wrong, can't walk down hill, bend it, stairs are a pain, but I have hope, as I am resting it and the pain is easing up. MRI is set next week, and with your amazing advice, I'll listen to results to avoid surgery and continue healing. Crossing fingers it's not scar tissue and inflamed nerves. Before I allow surgery, I'll consider your knee program once I have a diagnosis. Yes, steroid shot a year ago and recently, and I tried gel, but both failed after minimal relief for a day or so. My friend is ice, my pool, and rest. Still to angry to do mild kicking, so I just do small movements. You're the best and love your smart advice! And I caught that bit about water intake too and I've been on it lately!!! 😊

Hey Doctor David! I’m a 27 year old guy, and was very active in weightlifting before my knee cartilage injury. MRI came back with 7mm chondral tear. I did 4 weeks of PT (mostly posterior chain exercises) and it didn’t help much. My PT recommended I take 2-4 weeks of hardcore rest (no deliberate exercise, light walking only). I’m on week 2 and I can say my knee feels better, but then again, I’m also not on my feet as much. Should I keep up the rest and after 4 weeks slowly work back into very light bodyweight exercise? Thank you!

Dr David....you've been a blessing for me with your advice on cartilage repair and avoiding knee surgery. The OS tells me I'm bone on bone and I need knee replacements and I'm trying to avoid this. Watching your videos with the appropriate exercises but really worried about damaging my knees further. I'm 67 and regular with 10mins bicycle at home and pilates 3 times a week and a bit of walking. Dr. D, what do you do if you're job involves standing in one spot for 8 and a half hours? How can I take pressure off my knee caps in this case? I bend my knees slightly as I don't want to be standing completely straight either. What to do?

Hey Hansel783!! That's a tough one!! It sounds like you're doing the right things. You've got to see if you can still improve in your knee despite having to be on your feet for so long. You probably can do it. If your knee cartilage is too far gone, then you won't show an improvement and you'll have to ask for a job modification or consider changing jobs. Hopefully it doesn't come to that!

There is belt that helps you sit anywhere anytime. You can switch from standing to sitting very easily in few seconds. Try that.

I am an athlete, and I coach a few young children. My understanding of stretching the quads is while it causes pressure increase for 9 minutes per week, but it buys you the rest of the week of having less pressure on the knees. I have also had a lot of my athletes knee pain subside with stretching and quad exercises that target quad imbalances (and tons of posterior work). Is your advice in this video just to allow for cartilage regeneration? Or would you eschew stretching for anyone with knee pain? Doesn't stretching allow for less pressure on the joint in the long run? Thank you. I appreciate your videos very much.

Hey Paul, the research shows that the benefits of stretching only lasts hours whereas the benefits of strengthening last days. That's why we recommend strengthening over stretching - it's a more efficient use of time and energy for helping knee problems. Light stretching should be okay on the cartilage. But we find that people have a hard time keeping the stretch light. Also since stretching tends to be easier than strengthening they avoid strengthening. And that is what they need in the long term to fix the muscle imbalances.

Several years ago, while starting to exercise, I did 5 short squats while holding onto a bed post. Left knee swelled up so couldn't bend knee for 1 week. Since memory is short, I did that again: same result. The skin felt really "tight" around knee......so I decided to do 30 second stretches.....calf, quad, and hamstring each. This solved the problem. But, I'm noticing that "tightening" the glutes when straighten leg really does help. Great idea!

What a ton of new information. I need to totally rethink my leg use! No more quad stretches! I will miss those! But glad to know that squats are helpful . I can do them without pain. But usually the next day I have more pain. What about dead lifts?

Hey NAB, yes this can be a lot of contradictory info compared to what mainstream healthcare pushes. If squats are causing your more pain the next day then you're doing too much, or doing them too intensely. You need to also make sure your technique is correct so you make those glutes work. Hello Doctor. Thank you for everything you do. Might I ask, does your 6-12 month estimate apply to focal high-grade chondromalacia patella? My PT seems to think 8-12 weeks. I am 5 weeks in and there is good improvement from where I started, but I still feel pretty far away from being able to return to any moderate to high- level activity such as hiking or moderate weightlifting for my legs.

I’ll try to do 5 week updates for anyone who is like me and wanting to read through the comments to compare anecdotes similar to their situation. I’m now at week 10 and I am still overall far from where I’d like to be. I still sometimes have a bit of discomfort sitting, standing and getting up and down. Some days I feel almost perfect but then others I still struggle with these issues. I don’t yet feel ready to say, jog or hike, if I wanted to. The other day I flared up simply from walking in sandals for 100-200 yards and standing in them for 15-20 minutes on July 4th (sandals probably weren’t the wisest idea but I thought maybe I could handle them because I felt pretty good that day until doing that). However, there is no doubt I’m improving on some level because like I said, I do have some days where I almost feel fine, and I am now squatting almost 100 pounds with no pain or discomfort afterwards.

Thankyou Dr. David, I have an "angry knee" where the fluid feels stuck in swollen areas. Bursitis of some kind? I am going to try the knee swing exercise for each knee. Update! After 5 minutes of knee swings the pain is reduced I am almost limp free. I still have swelling but right after exercises, I had marked improvement... Almost no pain or discomfort. I also massaged my thighs to help release tension. More knee swing rounds to come for improvement! I feel very encouraged to watch more videos as I go to heal and strengthen the right muscles. Hi, I' am a physio from Germany. I knew about the importance av the glutes but I also showed exercise for the quadriceps to my patients. Since January I myself have problems with my right knee. Yesterday I saw your videos about how to walk and how to climb stairs + exercises for the glutes. I was excited and tried all of them with success = less pain. This guy talks a lot of sense. I've taken his advice on walking correctly, glutes, and scoops (on his video about pain in the inner knee). I followed the advice alongside what my current PT plan. I eventually got to see a specialist after an MRI. The specialist told me I would need a knee replacement. But after watching these videos, and listening to advice from people waiting, and people who have had the op (both on the Internet and in real life), I decided to postpone any surgery until I absolutely needed it. It's one of my best ideas, ever. My cartilage has been destroyed due to an imbalance in the way I walked. The podiatrist gave me some insoles to correct my gait. They hurt like hell at first. As did walking with my feet pointing out instead of inwards. I've gone from not being able to go up and down the stairs without using my hands due to the pain. To being able to walk 2 miles without it. I truly believe I can halt the progression, and maybe, reverse some of the damage, now I'm not putting all my weight just on the insides of my knee joints. When my knees first hurt, I stopped doing any activity. Was miserable. Despondent. Looking at the prospect of being in a wheelchair. That was the worst thing I could have done. You have to keep active. You have to keep mobile. So long as you do the right exercises (this guy, is amazing for the advice, I really recommend it!) One thing that really, really helped, was getting enough sleep. I went from 5 hours a night, to 8-9. It was a massive difference. Both in my mood, and my bodies ability to repair itself. I've nearly doubled the time my body can heal. Stress, avoid it. Be positive, it will help. I understand your worries. I had them too. In the end, I realised just how much I loved my knees, and how well they have served me in life. And rather than hate them, I loved them instead. I worked out, that having walked around 10 miles a day, on average. For 50 years. I've walked nearly 3 times around the Earth. Now that is amazing! Why would I hate something so amazing. Give them a little bit of TLC, and I'm hoping they can get me the rest of the way. Then it doesn't matter if they explode! ha ha ha... What an amazing bit of evolution. Truly.

Thank you so much! I had a serious fall on both knees. Cartilage damage in both and now Osteoarthritis in both. Talk about being in Hell with swollen knees and excruciating pain where I couldn't even walk. I was practically dragging myself around on the floor. It has been over 8 months now. 8 months off work. On the waiting list for endoscopic surgery to shave and clean cartilage. Which I do not want to do. I went to Physio and all the exercises you said not to do, my Physio made me do including resistance bands with the knee extension and weights with squatting. It was painful. No matter how many times I told him, it was causing me great pain. He treated me like I was some kind of wimp. I did martial arts for over 20 years! My knees ballooned up and I couldn't walk again. After that I stopped going to Physio , wrote him a nice e-mail saying I was stopping and still he insisted I had to continue weight bearing exercises! I knew those exercises were stuffing my knees up but wanted so much to believe in a professional. Thank you for your advice, hopefully somehow my knees will get better ❤

HELLO FROM CALGARY ALBERTA CANADA. like you said, I was told to strengthen my thy muscles. So I went swimming and used the fins, for an hour which made my knees worse. I found using Tumeric, which I cook into a paste with a little Black pepper, and extra curcumin I add after, seems to help a lot . But the swimming with Finns worsened it. I have osteo,.and was told after Xrays that my knees are really bad. I told them I wanted to do everything else first before getting a knee replacement. I walk every day, for 2 KM. THANKS TO OUR DOG, and Tumeric, dangling, with weights, once a day. but I will do more of it, as you said. So thank you for your info. Sincerely Holde.

The bridge is a GREAT glute exercise! I can't feel my glutes when I do some of the Glute exercises. It seems like working on the hamstrings should balance out the muscles too with the glute workouts!
