The Russo brothers said that RDJ is playing Victor Von Doom so definitely it is not a variant of Tony Stark .I think if they do enough make up and stuff(without beard with his face fully burned and he only takes off mask for max 1-2 min in entire screen time) , he can differentiate his look from Iron man.Also RDJ is a phenomenal actor if you have seen Oppenheimer you know , so he can definitely pull it off.He can be Doctor Doom of Fantastic Four Universe as that universe is very different from earth 616. They can show in the post credit scene of F4 Movie that he somehow escaped when his earth got destroyed by Galactus. Later after Secret Wars they would most likely recast him as he is only there until Secret Wars. In comics his face became normal right , so here they can show as if his face has entirely changed . Then we might see Cilliun Murphy or any other talented actor as Dr Doom

I have a fan theory: In Doom'sday they start the movie with RDJ's Tony stark (in another universe) same as 616, same behaviour, same look, same people around him, same story, just that he killed Thanos in endgame and got out alive. Then another universe Doom comes in the hunt of power for Tony stark of this universe cz this Tony is kinda supereme cz he killed Thanos. So Doom manipulates him or something like that and use him like he used molecule man/Dr strange in comics. He(doom) absorbes his(Tony) power and knowledge and everything then become undefendable and start to look as Tony also.

In the earth 616 Where we see that in the first iron man movie, that Tony get an arc reactor when his chest was destroyed, but may be in any alternate universe or time line may be Tony face was badly injured by the terrorist attack and like doctor strange his face can't be recovered and to fix that hee will go to Kamar Taj like doctor strange, form where hee will learn magic. And may be by any circumstances he will change his name to Dr. Doom, and that will be the origin story of Dr. Doom, mention me if you like it

it would be better if it start as, in alternate timeline iron man wasn't able to grab infinity stones from thanos's hand then thanos snap and again vanishes half of the population but tony stark still alive. the scenario is these infinity stones wasn't belong to their universe and anvengers will not be able to retun them then TVA came in picture and start vanishing the effect but they finds out the iron man is an anchor being of their timeline so they can't vanishes him. Tony stark flew away from the scene then he merges his time travel technology with the help of AI to combine it with his suit and starts visiting multiple time line to defeat thanos but this creates chaos and soon after he becomes DR DOOM and one day he arrived in main time line of the marvel.
