Swan neck finger positioning, occurs due to an untreated mallet finger or can occur with rheumatoid arthritis. Swan neck positioning involves hyperextension of the PIP joint and flexion of the DIP joint. I believe you are referring to ‘intrinsic plus’ where the MP joints are flexed and IP joints are at neutral or 0°. Research has shown this is no longer necessary and people regain their finger motion following splinting with the fingers in a position of comfort. There are occasions where the hand surgeon would want intrinsic plus positioning, but these are less common.
