ABOUT OUR CHANNEL
Hey guys my name is Larry and I am Critical care RN. I currently work in a Cardiac/Vascular ICU and have been there about 8 years now. I really enjoy getting to be able to help people and their families get through undoubtedly the hardest times of their lives. I am married to my beautiful wife Wendy and we have one cool little dude named Jacob! We are both nurses and she really inspired me years ago to do what I’m doing now. My goal here on the Critical Care Connection is to educate nurses on different topics and give them the knowledge to be at the top of their game everyday! Hope you guys enjoy the channel!

She flushed it without removing the tourniquet first. Always remove the tourniquet before inserting anything into a vein.

Those are the exact veins that I have trouble hitting. You can't necessarily see them in that location. Thanks for the video.

Do one for edematous arms and elderly patients with tiny friable rolling veins and loose skin!

Hey Claire! So you ask one of the most problematic questions in all of IV sticking land. Don’t feel discouraged about not feeling them because there are tons of patients that don’t have great veins. Depending on what subtypes of patients you work with, you may find that you’re constantly getting the harder to stick patients. I’m glad you brought this up because me and my wife were just going over a plan for a follow up video that addresses this very issue and how to use different techniques to increase your success chances. I think it will be just what you’re looking for too!!! Hope fully we should have it done and out within a week to week and a half. Now when you can’t feel the veins are you able to see them at all??

Great video though bit of advice to your viewers--always pull that tourniquet before you pull the stylet. Otherwise can be super messy!

In addition to “advance a little more into the vessel“, I always teach my young colleagues to not retract the needle, but to only advance the plastic. This gives the plastic more stability for advancement.

How come? Are we not supposed to retract the needle to ensure blood before further advancing the catheter?

You are taught to retract the needle some for return. As you can tell in the comments there are lots of ways and opinions on the best way. I think the not removing the needle while advancing thought is suppose to be in the case that maybe the plastic portion is only partially in the vein and to keep it from just bending. That’s my best guess on that but if not careful you will take that needle right through the other side of the vein. 😬. So that being said…. Proceed with caution

My problem is finding a good vein . Often I feel nothing any advice? I tie a tight tourniquet etc….

How are you looking? Only with your fingers, or with your eyes too? Some people won't stick veins they cannot palpate, which means you are missing out on perfectly healthy vessels. Some people, especially people who are chronically ill just don't have a lot of selection, that is where Ultrasound guidance comes in handy.

Hey Katie! Those particular IV’s did have a valve to stop blood flow from coming out which is super nice. I have done my fair share of IV’s without though and it does add a different style to your technique. Once you get flash back, the hand your holding the skin tight with will need to slide and inch or so above the insertion site and occluded blood so do it doesn’t bleed back out. You may want to use your pointer finger to hold the catheter still and pinkie finger to occlude pressure. We are putting together another IV video that I think we be helpful to you. I have ordered some non valved IV’s to show how that will look. I don’t know that it will be here before your first time but it will have difficult stick techniques too. I wish you the best of luck on your first day!!!!

What is the best angle to use when inserting your iv needle

Hey good afternoon!! I usually start at about a 45 degree angle and as I feel like I’m getting close or even when you get flashback if you hit the vein early then you start dropping your angle flatter as to go more in the vein vs. going through it. After a while your hand will just start doing it naturally in a smooth curve motion.

@@CriticalCareConnection 45 is way to steep unless you are using ultrasound. I would say 20 degrees and vary slightly depending on how superficial it looks.

My Aunt went into the hospital for open heart surgery and came out of the hospital without her left hand bc her iv infiltrated and no one would listen she even told the house keeper so sad

I'm so confused... is this iv set have an auto closer? Why isn't the blood flying out of the end?😮 please help , I have to start my first iv in 2 days.. in new pre op job! 😊

The catheter is probably a blood control catheter, which won’t let blood flow out as soon as you remove the catheter. I use BD Insyte Blood control catheters not sure what brand these are. They are very useful for blood safety and not having to hold tamponade.

Hi Katie! How'd you do with your 1st IV? 99% of the time the blood will drip out of the hub but in this case the fact that the tourniquet wasn't removed prior to pulling the stylet should've increased those odds to 100%. Was surprised it didn't :)

@@lucidmed3343 it's a valved catheter, so the blood will not come out unless it is purposely pulled out once an extension set is attached.

@@bettysmith4527 Hi Betty, valved catheters are helpful in that respect however I'd still get into the habit of removing that tourniquet as soon as I confirm placement regardless. If not using a valved catheter for whatever reason it will make a complete (and unnecessary mess). It was left on through the placement of the tegaderm. I understand the priority is to secure the line but a one handed release makes quick work of that. D

Hey good question! You want to remove the tourniquet after you successfully insert your IV into the vein. Depending on the style IV you have may make a difference too. Some IV have a valve that won’t let blood run out after you stick so it makes it easier to do other things like remove the tourniquet, or hook up your tubing. Some older IVs don’t have this valve and you have to hold manual pressure to keep it from bleeding while you do these things in that case. If you remove the tourniquet last you wouldn’t be wrong. That way you didn’t bleed everywhere. Hope that helps.

Some brands do and some do not. I think it’s just preference. I believe those are Braun.

These videos are interesting, because federal law restricts the use of certain medical supplies such as IV catheters. This info is found on the box (sometimes in the form of “Rx only”) and is the reason an order is required for placement. This applies to the start kit, IV itself, and saline flush. I would hesitate to post a video showing my face using restricted medical supplies in my home or office in a non-emergent setting off the clock. Practicing medicine without a medical license is grounds for forfeiture of an RN license and further criminal charges.

Interesting perspective. These supplies can be purchased by anyone, and were the same ones given to me in nursing school. I’m using them for the use of education and certainly not practicing medicine. An order is not required for placement of IV’s at all facilities. There are many people that found this content helpful in their practice. I’m sorry you didn’t get the same out of it.
