This is for those who have the same problem. I am 21 years old and my father is a gov employee. But all of you who has same situation as mine need to enter NO in "is either of your parent (in case of minor)/spouse a gov employee. In such case enter NO in the above field and hence you do not need to fill any annexures. Today I just visited PSK and the verification has beed successfully completed. I am applying fir my aunt and she is teacher in state government school in bahraich and i want to know that these government school don't have letter head and this form shoukd be attested by only principal or from the DIOS office also verify this and after verification we take photocopy of that original form and the babu sitting in school they don't have any reference no so how it can be possible. Thanks for your complement. If letter head is not available go for annexure g or h. Even there is rule if letterhead is not available one can apply in plain paper but better you take annexure g. Good luck. And let me know after your aunt get passport.

Just download annexure g form from passport website. It is a simple form fill it and sign it from the Dios. There is nothing tricky but few passport seva kendra official may not accept a plain paper annexure a.

Normal print is acceptable if the office doesn't have letter pad. Simply your dad's ID proof will not work. Take annexure a with normal print with office name, logo and address on header. 👍

Thanks for your appreciation. You have to paste 2 passport size photo graph on annexure a one on each. That's it, no need to carry extra photo graph as they will take your photo graph in dslr at passport seva kendra.
