Just write where you schooled on the application form and place all your grade 12 certificates as documents and include where you improved in your CV

At school just write the school you completed your grade 12 and the subjects (don’t forget to write your majors) Then in your CV indicate at education: the school you attended grade 12 and where you improved and make sure you attach all your certificates
