“How are you?” is just a conversation starter, be natural, confident and polite in whatever you usually say to other people. I have given many interviews and my standard reply has always been: “I’m doing great, how about you?” Never felt as if interviewer didn’t like that response. Remember, both of you are professionals, I am sure you wouldn’t give an expanded answer if your colleague asked you the same question, similarly the interviewer is also used to hearing that normal reply.

Also, you should always try to build rapport by engaging in small talk in the beginning of the interview. More importantly, keep the interview interactive rather than a mere Q&A session and one should NOT make it look and feel like a typical college viva-voce environment. After all, a candidate should always look out for ways to stand out from the competition :)
