I've followed this code exactly and always it only inserts the first row, never the others. The values are being read from the input boxes. The sql shows the multiple values, but it only just inserts the first

Instead of a variable $phone[$index] use $_POST['name'][$index] directly like this foreach($itemtype as $index => $data) { $itemtype = $data; $size = $_POST['size'][$index]; $color = $_POST['color'][$index]; $quantity = $_POST['quantity'][$index]; $notes = $_POST['notes'][$index]; Rest of the code... }

if you mean fetch data from dropdown(select-option), you have to create an onchange function to fetch data in another table ($get(url)), then the value obtained is put in the input element with jquery
