This is more on query builder rather than eloquent. You'll just using eloquent to get the table name. This is more eloquent when you used eloquent relationship.

I'm just trying to link the customer (form is filled out with their contact info) to a survey (after the form is filled out, it takes them to a survey page). that's DB Query Builder, and it's completely different than Eloquent. Also you don't show the JSON response from your controller, it might be interesting.
