El perno quedó sobre el material?... Es recomendable o debería hacer contacto el perno con el diente? Lo pregunto por qué al prepararlo no queda ese espacio. Generando dos cosas, a mi parecer, una un tamaño mayor y dos una probabilidad de desalojo de la pieza. En la radiografía es claramente bicible el espacio virtual y en el vídeo igual

Pocos dentistas hablan del perno muñón, la mayoría prefieren arrancarte todo el diente para después colocarte un implante, cuando ya tenías uno de forma natural! Habría que publicar más todo esto para que todo el mundo supiese que en cierta forma algunos no hacen el mejor diagnóstico para tí sino para el bolsillo de ellos!

I always seem to struggle to get a good amount of length when making my post space. First thing I do is melt out the Gutta Percha using a heating tip and then either using a gates football or parapost drill tip I hit a dead wall where I can't go any further and out of fear of perforation I don't force it. So was wondering what are the best tips to use on a latch that works the best? Thanks

Hello, what you are doing is good however, you should have drills with multible diameters such as gates glidden drills (1-6) in which #1 is the smallest while #6 is largest. According to your story if you hit a rigid wall before reaching the intended lenght this indicating that the size of the drill is lardg, therefore you should stepback and use a smaller drill, if you hit a rigid wall again step back once more and so on untill you go down the proper length without resistance.

Any feedback from dentists on use of glass ionomer to reglue post and crown. I struggled to find dentist in Thailand that had this. Lasted 4 years but needs reglueing again. Also ideas on fine fracture way deep in that is being ignored at this stage. Does the glass ionomer support the fracture at all? And how well does glass ionomer stop decaying from occuring. Last one. is blackening of the tooth structure a cause for concern or purely aesthetic. Cheers . Merry Xmas all

This might be the more traditional way of doing it, but I think that it has many advantages and is much underused for making cast posts these days. The advantages are accuracy, less material usage and associated cost, well tolerated by the patient.

And they are solid as hell. You can't remove this even if you want to. With proper endodontic treatment and assuming that no root fracture occurs this will last forever.

@@TheFinalCutBG it takes 2 visits, very time consuming, requires a lab and a lab bill, excessive tooth structure is removed, more people and factors involved leading to more errors, and so many of them are short little non retentive stumps. With today's options, this is a ridiculous as well as inferior way to do a post and core. But, if you like it, go for it.

@@ronyeahright9536 sometimes when you don't have enough healthy structures for composite build up, this method does miracles. However I don't make the wax model myself, I just take the impression and send it to lab, but with a good dental technician, this core is a very solid base for a crown, even if you don't have enough ferrule. Edit: I use this in case of severely compromised teeth.

@@TheFinalCutBG I was taught this method in school, it uses Duralay resin to make the pattern. I have also done the technique you mentioned, of taking an impression and having the lab fabricate the pattern. I have found that a prefabricated , high quality retentive titanium post such as a larger size Flexi-Post red, #1( gapped in the center so it does not split the root) cemented and then built up with light cure composite is the best way to restore these teeth. Fantastically retentive, you eliminate the extra visit, you eliminate giving over control to a lab, and you eliminate a lab bill. If you've never used Flexi-Posts, give them a try sometime. But good luck in your technique, it sounds as if you have mastered it well.

No, custom made post and core is not outdated and still has its uses, at least it's one of the treatment options for any dentist to chose in certain cases.

Azhar FPD Dear Colleague At the age of composits cast post and core show many drawbacks: It corrodes Il is opaque and is not suited in case of full ceramic crowns They are 15% cases of allergy due to some alloys Also, originally this technic was designed with high content gold alloys. With the increase of gold cost, many practitioners choosed non precious alloys as an alternative solution The fitting of non precious alloy is not satisfactory because of cast shrinkage; also corrosion of non precious alloy leads to swelling of the post which acts as a wedge causing subsequent root fracture In the other hand composits appear to overcome all the despointments encountered with metal cast post and core especially when using laboratory light and heat cured composits materials with glass fiber or carbon post Contrarily to metal post and core I seldomly observed tooth fracture or post dislodgment Another advantage of composits crown buildup is that it is possible to process the post and core at the same session of root filling stage Thé littérature has shown that most of root canal therapy failures are caused by bacteria invasion at temporary filling stage I always process the crown buildup under rubber dam at the same session immediately after root canal filling; this is not possible when using a cast metal post and core nowadays, cast metal post and core should be considered as a professional mistake Sorry for being so rude; Dr Denis Adrai, DDS, Msc in Prosthodontic University of Pittsburgh; Chairman was Pr. Simple but not reliable. Intraradicular part shouldn't be round and contact area between post and tooth must be on different levels to prevent shifting.
