You can use blank value on Reset and Formula on calculate button. Just assign the blank or relevant formula on calculate button as per requirement.

Whenever we create option buttons then Excel keeps all option buttons in one group by default. Suppose you have created four option buttons and out of four two are Male and Female & rest of the option buttons are Smoker and Non-Smoker. Here, Excel will keep all in same group. To make it separate, you need to go to Option button properties and give the relevant Group name. Here, you can give 'Gender' as group name for Male and Female; 'Smoker' for Smoker and Non-Smoker option buttons. You can use the same code after changing the option button name for next group.
