The stethoscope's diaphragm is placed over the lower end of Brachial artery not the vein, though there will be veins superficially just above the brachial artery or its bifurcation. This is the sound of passage of blood from brachial artery which is measured. Second the correct method is recording on right arm of the patient.First palpatory method should be used to check upper limit of systolic Blood Pressure with fingers on radial artery without stethoscopte and then auscultatory method using stethoscope. It is not necessary to inflate the cuff to 180 mmHg but that depends upon the reading of Systolic BP through Palpatory BP for example if the systolic BP through Palpatory method is 220 then inflate the cuff to around 230 or above and then start deflating or if the Systolic BP is 130 thtough palpatory then inflate cuff using stethoscope to 140-150 and then deflate.
