For operative report #8 for N40.1 why isn’t the additional code (N13.8) for the urinary obstruction coded

N40.1 is assigned instead of N40.0 because of the urinary obstruction; however I will add N13.8 for urinary obstruction as a secondary diagnosis.

@@codingwithhoang As there is an instructional note at N40.1 stating "use additional code for associated symptoms when specified", since it was specified I would add it. Your video are amazing, thank you!

I want to thank you for all your generosity and knowledge you have impacted to my life, I passed my exam last year, and to God be the glory have gotten a job as certified medical coder. Sir am planning to migrate to Uk, I realized that their coding systems is a little bit different from WHAT HAVE KNOWN. THEY are using icd-10 and. Opcs( operating procedure codes.) sir, I will be excited if you can do a video on OPCS that is recommended by NHS. Wait to hear from you soon .

Good morning Rasheed. Congrats on your new job as a certified medical coder. No matter what classification they are using, the data they collect is still based on either the CPT or PCS for the procedure, ICD-10-CM, comorbidity, coding rules and guidelines for the diagnosis. You will be fine. If you are new to inpatient record then focus more on the PCS inpatient procedure codes. Good luck!

I have been waiting for you to upload another video! I do have a question. With Op Report 9. How was I supposed to know if the hernia was recurrent or initial? I tried reading through the report and was not able to get that code. Instead, I ended up with 49593 (initial hernia, 3cm to 10cm, reducible) :(. Correct me if I am wrong if the op report does not disclose whether it is initial or recurrent are we supposed to code initial?

Yes, when a patient has no history of hernia and the op report does not indicate whether it is initial or recurrent then by default, it is considered an initial hernia. The postop diagnosis states "recurrent ventral hernia" so why you ended up with 49593 for an initial hernia? Also, when the old mesh was removed during this episode then it should give you a clue that a hernia repair has been done before. I hope it helps.
