You should look at the Petzl Nest, much simpler and has a spreader bar already built in for easy transfer from horizontal to vertical.

Why did you wait until the patient was lowered over the edge and under load before clipping the spreader bar webbing into the carabiner (about the 5:45-5:55 mark)?

Because if you notice as they began the lower, if the webbing had been clipped in, it would have prevented the stokes from dropping flush against the wall as the first half of it went over. The idea is to have it slowly follow the ledge over until its vertical.
