The guidelines on the website mentions that the "prescribed performa in duplicate with signatures" is needed. You can publish advertisement online or you can go to your nearest newspaper agency. In my case I apply online advertisement through Releasemyad website then select the date on which you want to release your name then go to any news vendor and buy 5-6 copies.

Hi Akshit, very nice video. You have explained each step so well. I had query though: 1. should the affidavit content be printed on a4 sheet and notorised? Or on judicial paper (the green colored ones) 2. The content in your affidavit looks a bit differently worded than in the pdf i downloaded from the gazette website. Which is proper? Does the wordings matter? 3. Should your photographs be pasted to the request letter while sending via post? Or should i keep it seperately? 4. How many passport photos to be submitted in total?

No don't post anything on request letter, they will themselves paste it. Also you can type affidavit text on an A4 sheet but make sure it's notarized + attested by SDM

@@vin_v Yes adding surname will need duly attested affidavit by IAS. It's not a big deal to get one, just go to your local SDM office and ask him to duly attested the affidavit and put his stamp on it. It took me 15 mins to get it done.

Hi Akshit, thank you for the informative video. I have a different case here, I don't have a surname in any of my documents and I want to adopt my husband's name. I added the surname in Aadhar and Pan but now I have a passport re-issue appointment to add the surname. Would I need a Gazette notification? I don't have my husband's name on my marriage certificate. It is just Reshma. I am really confused. Waiting for your kind reply :)

Bhai sunno tumhe kahi jaane ki zarurat nahi ekk affidavit banvao Aur notarised karvao bas 100-150rs mein ho jaega aur 100rs ka bhi stamp paper zayada ho gaya 10rs vale stamp paper se bhi ho jaega aagar aalag surname karvate ho like family mein sharma surname hai but tumhe singh karvana hai jab Magistrate ke sign chahiye hote hai. But tumhe remove karvana hai toh ekk affidavit banvao 10rs ke stamp paper pe Aur notarised karva lo aur newspaper mein ad nikalva lo phir gazette published karva lena sab smoothly ho jaega.

With Gazette of India and Aadhaar card, you go to Aadhaar card center and where your name is printed in Gazette of India, sign there and it will be done.

When name is updated in Aadhaar card, after that you apply for pan card online for name correction and attach adhaar card and gazette copy in it.

Bhai I prepared my affidavit like yours. There is nothing written on the e-stamp paper, I have rather typed the affidavit text on a regular A4 sheet and attached to the e-stamp paper. Also the IAS in my city only duly attested the front e-stamp paper not the second page on which I have written the affidavit text. Both pages are notarized though, the SDM said you don't need his seal on both pages just the e-stamp paper. Is it ok? Or we need IAS seal on both e-stamp paper and affidavit text paper ???
