Download the zip file of this tutorial then extract it.. After extracted copy this folder and paste inside htdocs folder then open XAMPP and start apache and mysql then open your browser and type localhost/foldername/filename.php

In the html code of the forgot password page there is an input field and it type is number. You've to change the number to text.

You can't decrypt password_hash hashes. But you can easily decrypt md5, sha hashes from many free password crack sites.
