He didn't snug the wire to the bolt head. The book had lousy photos, and same for his work. It's floppy around the bolt head.

AC 43.13-1B and the General handbook FAA-H-8083-30A both say to make the wire taut and not tight. On the practical test I always have applicant hand safety wire and not safety pliers are used.
