Hello if the reader is a family doctor, is it a known case or an unknown case (oet writing). family Dr same as GP

Hi, a family doctor would be similar to a GP and you can assume they know their past medical history but it might be better to include anything which is essential to the purpose of the letter just in case, such as like this: "As you may be aware, Mr Jones had been hospitlised several times before for acute infections...". Please how do we interpret Nursing Management case notes? i have a problem of mixing them up. when I take it as care that was given to the patient during hospitalization, it turns out to be care that should be given after discharge or refer and when i interpret it as a care thats supposed to be given during admission then it appears to be care that should be given after discharge. can you or anyone help me with a hint please...
