I’m in nursing school right now and had ER clinical last night. It was a quiet night and a well staffed ER. One of the nurses not much older than me let me do my first ever IV on him. He had good arm veins and wasn’t phased by my lack of experience. It was awesome!

We have different IV cannulas, but once I initially poke someone and get flashback, I wait (count to 2), then advance the whole cannula (needle and all) to open the hole in the skin a little more, then advance the plastic tube (catheter) and withdraw the needle. Too often I think new nurses try to be too quick and don't open the skin access that little bit extra, so when they advance the catheter it kinks on them. The patients will quite often have a fight or flight reaction from the pain, and their vein can collapse, being patient allows the vein to refill before you try to advance the catheter. Speed comes later, skill first :) Also, don't be afraid of the tourniquet, tie it like you mean it. A loose tourniquet doesn't do anything. I also like to explain to my patient what to expect ie/nasty poke here, just give me a second, I know that tourniquet is tight, communication goes a long way!

But veins don’t go in absolute straight lines. If you advance the needle further than 1-2mm… let alone the whole needle as you mention, you will likely just puncture the vein
