The double first knotting cn be achieved using the same one handed method.... i used a double loop of the suture end further away from me and then pull through just like the one handed method and the knot locked same way

Your videos are great! Really helpfull :) I got a question.. What's the difference between a surgical knot and the instrument tie? Are they both equally apropriated for any situation, or does it one has it's own use? In the middle of this video, you do a double loop, to get more tension on the line. This is similar to the double loop done in the instrument tie?

Yes, the double loop can be done in both hand surgical tying and the instrument tie. It's not always necessary during hand surgical tying because you are able to hold continuous tension with your hands in most situations, but it is helpful when the tissue you're trying to approximate is under a lot of tension. We generally always through the double loop as the first throw of an instrument tie because we're not able to keep tension the same way. To answer your first questions- We usually use the instrument tie when closing skin (although you can use your hands) and the hand surgical knots are used for internal or deep suturing, generally in the OR. I hope that helps!

Do you have to already have the suture cut or can you use this while still having the needle attached to the longer end?

ALWAYS remove the needle!

The needle can absolutely stay. Many sutures require it, even. Just protect the needle tip with your needle driver.

Yes you could, but just need to be opposite direction. Use index and middle finger to chop up, grab opposite suture between index and middle finger to pull out of loop. It requires a bigger loop and could be a little awkward. Best to practice using just one finger

This may seem dumb, but the white bit of this practice kit would be attached to the needle in real life, right? Or at least, one of the sides. Does that make a difference when pulling through? TIA!

Red is the needle side. Its the side you don’t let go of so the needle is under control whilst you tie.

It's a "one hand tie" in which non-tying hand can just hold the instrument, while the tying hand does all the work. This is in contrast to "two-hand tie" in which fingers from both hands need to move
