How to Tranfer Pokemon from Gen IV to Gen V

How to transfer Pokemon from Heart Gold/Soul Silver/Diamond/Pearl/Platinum to Black/White 2 using the Poke Transfer Lab.
Note that you will need to visit the Move Deleter and delete any HM moves before you can transfer the pokemon and all items they are holding will be returned to your bag. Also pokemon wont evolve using this method or Pokemon Bank.
