So for example: "found family" would be a topic not a theme, but "found family is better than blood family" would be? (And yes, absolutely second the idea of finding and or refining your theme in revision!)

What is the difference between a theme, a story, and a plot? Im having a very hard time finding satisfactory definitions for the three that don't overlap. Im looking the exact point at which they are seperate from eachother.

A theme is: the philosophical stakes of your story. E.g. is it possible to outgrow your past? Or Does the ends justify the means? A plot is the core events of the story. E.g. A warrior is faced with xyz terrible choice in order to save the world. The story is everything. The prose, the plot, the theme, the characters, everything all together.
