How were you able to build most of this is it the happy home paradise? or is it because your island is more than three stars? I get so lost when it comes to designing my island and wonder when I can achieve something like this or if it’s a mod?

Nope no mods at all! Once you get to a 3 star island you can unlock terraforming in order to build cliffs and rivers. Then it’s a matter of collecting the right items and custom design codes! I find a lot of the hard to find items on Nookazon and I search on Pinterest and google for my custom codes! Hope that helps!
