Hypothesis: "First to out" is a false meta. I think "first to out" happens more rarely than if positions were assigned randomly. The only reason people thinks it's typical is because you notice the person who wins the round, so you notice every time it happens. It's purely a product of the frequency illusion - confirmation bias!

I think it's a real thing, the same happens in fighting games. Sometimes your brain goes into overdrive and you absolutely crush your opponent, and then the next game you can barely play and lose. Sometimes when playing friendlies with a worse player, I'll be winning every game, and then they'll play really well and take one, and then invariably they go back to getting destroyed every game because playing your peak is just more tiring than autopiloting. The moment they win a game I know I'm winning the next one.

I think it's a real thing. For example Massa always plays very risky in CotD. So he either finishes near the top, because he uses the fastest lines, or he crashed out and get eliminated. So it's all or nothing with those players.

@markgeers8488 If you had to estimate how often "first to last" should happen if it was random. How many times would you say it "should" happen, and how often do you estimate it happens in reality? Let's say we only count first to last as being valid down to the top8.
