His reaction mainly led me to believe deep down he knows it was a clean shot. But I could be wrong. There are shots that clip the cup and slam it into the jewels, without the punch landing flush on them. It could have been one of those, you can see the garments lift in the slowmo. Definitely with in the margin for a low blow as most of his fist was below the belt. Also depending on the type of cup, a high low blow can still catch the top of the cup causing the cup to violently jolt upwards, hitting the testicles.
