Holy crap, this game is based on Johnny Castaway, a Windows 3.1 screensaver. I thought the setup looked familiar, then I saw the name of the game was "Johnny's Island" and googled images of the screensaver to make sure.

Fun fact, him not losing weight while being stuck on an island is probably more realistic. When you start starving, your body holds onto as much fat as possible, because that helps you survive longer without food.

Yes. This man very clearly has a condition called Kwashiorkor, caused by malnutrition, particularly lack of protein. It causes fluid to be stored where it shouldn't be, which causes fluid to enter the tissues, causing life-threatening issues and that very distinctive bloated stomach.

Actually when you starve you don’t have enough nutrition so your body starts eating the fat for the nutrition it needs :)
