Probably your game is running WAY above 60 FPS, so things like these happen very often, specially notable on mammoths (for some reason) or in physics props like signs, those pushable wood carts, some dropped items even, also the high fps cause traps like boulders cause a shit ton of damage

Similar bug - when you fast travel to Sabjorn (Honningbrew Meadery), but system put you on some upper level and you do death fall down. CURE: fast travel to Whiterun Stables and go to Meadery by yourself.
