How should coders interpret OCG I.C.13.d.2 ? A code from category M80, should be used for any patient with known osteoporosis who suffers a fracture, even if the patient had a minor fall or trauma. Is it necessary to query for all patients with osteoporosis and a fracture? Can coders make a link to pathological fracture based on the OCG? Thanks you! 😊

Hi! OCG I.C.13.d.2 is titled "Osteoporosis with current pathological fracture". This guideline is specific to pathological fracture in a patient with osteoporosis. If the patient has a diagnosis of a pathological fracture and has osteoporosis, the coder can link the two conditions and code to category M80 Osteoporosis with current pathological fracture. This is because the alphabetic index links osteoporosis and pathological fracture by the word "with". It is important to understand that this link only applies to a pathological fracture. It does not apply to a traumatic fracture because the index entry specifically says "with pathological fracture", there is no entry for "with traumatic fracture" so the coder cannot automatically link a traumatic fracture to osteoporosis in ICD 10. If the physician has not clearly stated that the fracture is pathological then a query may be needed to clarify if the fracture is considered by the physician to be pathological vs traumatic. M84.4 Pathological fracture, not elsewhere classified is for fractures specified as pathological (due to a disease process) or chronic but there is no further information in the record to allow a more specific code to be assigned. It would not be used to often in the inpatient setting because typically information for a more specific code would be available. M84.7 is for Atypical femoral fractures which are a form of stress fracture that is associated with osteoporosis. These fractures occur in patients receiving long-term bisphosphonate therapy or other medications such as glucocorticoids. The entire term "atypical femoral fracture" must be explicitly documented in order to assign a code from subcategory M84.75. (Reference coding clinic 4th Q 2016 pg 41)
