Hi Tom. I've seen some amazing results with these implants. I've never worked with them personally. However, I love the idea of using a small implants to avoid difficult, lengthy, and expensive bone surgeries and restorations. Go for it!

Hi Dr Jazayeri. I I enjoyed hearing your information! Question? If you have a healthy lower second molar and jaw bone, but had to have the tooth removed due to a crack down the middle of it, would it be better to choose the delayed-placement procedure because molars have more than one hole root structure holding them then their anterior teeth counterparts? My reasoning would be is the dentist, upon healing in 4 months, has an exact, solid area to drill in and place the delayed-implant, as opposed to trying to fit it in, in an immediate-placement between the two roots of the molar and not having it accurate or secure and then patching it with immediate bone graft hoping it lines up and is secure in 4-6 months for the new abutment. Your advice would be nice to hear. Thank you in advance. Sincerely, Paul.

Hi Pei. Great question. The primary difference between a good and bad dental implant comes down to the thread/screw design and the screw material (the actual consistency of the Titanium alloy). In my experience, a good dental implant brand has a 96 to 97% success rate in the hands of an experienced practitioner. On the other hand, a poorly designed implant system can have failures as high as 20 to 30%! So yeah, it's worth it to invest in a good implant brand. There are a few dozen implant companies out there. Luckily, there are a handful of good ones. I'd rank the top dental implant brands as those made by Nobel, Straumann, Zimmer, Biohorizons, and Dentsply. Hiossen, which is Korean, makes some good, budget implants as well. Doctors, please share your experience as to which implant brand you prefer (and which ones you'd avoid!)?

Thank you so much for making dental information accessible and easy to understand for patients like me. I wish your practice is in my area. May I ask how long can one delay getting an implant after an extraction? I have a bad tooth decay on an upper molar (#14) and the cavity has hit the nerve. A quater of the tooth is gone. I was debating between getting a RC vs an implant. If I was getting an implant, I am thinking to get an orthodontic treatment first after the extraction, then come back for an implant. The ortho time would take 2 years. Would that be too late to get an implant afterward? Also, if I was in pain for 2-3 weeks, and during an consultation, my pedodontist did some tests and tell me the tooth is pretty much dead, is it worth it to do a root canal? Does the infection pretty much spreads around the body by now? Thank you 🙏.

Should all the conditions for a successful immediate implant placement exist for tooth 19, will an oral surgeon do an immediate implant placement on a molar?

Hi Frusci. Sure, you can do immediate dental implants on lower molars. The area has excellent jawbone which is great for implant placement. The challenge to immediate implant placement in the molar region is preserving sufficient jawbone to lock in the screw. You must perform a gentle extraction (atraumatic) to preserve the jawbone. If the little piece of bone between the tooth roots breaks, you can not perform immediate placement. You have to graft, wait, and place the implant in a few months. I usually cut the tooth to avoid fracturing the jawbone when I'm planning on placing immediate implants on molar teeth. But yeah, it is difficult and very technique-sensitive. Only experienced oral surgeons/dentists use this method, especially when it comes to molar teeth. Best of luck with your treatment!

Hi there. Great question! It should be the same as any other implant technique. However, this only applies if all the conditions are met. In reality, there is a slightly higher chance of implant rejection with immediate placement. Immediate placement is very popular for complex treatments where we remove dozens of teeth and place 4 implants for all on four treatment. It saves a lot of time and cuts down on the number of surgeries. Overall, I would say dental implant success rate, assuming a good implant brand and an experienced surgeon, is around 95-98%, then immediate success rate would be around 90%.

I have a question. I visited 2 different dentists to get info on getting implants. One recommended just two individual implants in a row and not the third cause there was no bone and he was worried about failure. The other dentist suggested a bridge implant so I'm kind of confused on which route to take. The implants are my top 3 back teeth all in a row. There haven't been teeth up there for years. Any suggestions?

To anchor the implants, you dont need lots of bone. But for esthethics, and better "grip" later, you do add the bone graft around the implant(s).
