What is the formatting in Word for a fill-in-the-blanks question (singular and multiple)?

I use notepad to compose my questions instead of word so that no invisible control characters sneak into the text.

Thank you. Excellent tutorial. It was the only one I found so easy and clear. Just a question: the questions don't go up in the enumeration and randomization formatting, would there be a way to do it in batch?
