I finally got to witness the ol' surgery vs. medicine struggle in real life. I was floated to an ortho floor on night shift, looking at my patient's chart, and noticed one of my patients was "off-service" despite being post-op for a hip fracture. Her blood pressure was 150/80. She was transferred to medicine...because her morning blood pressure was 150/80. No consult, just a straight transfer. I could feel the medicine attending's defeat in his short note about the patient's normal assessment findings and then finally "pt admitted to medicine for mild post-op hypertension."

This is why I personally feel IM has the second highest burn out rate: it is the disrespect and the way other specialties just dump the cases they don’t want to deal with on them. Exactly why ER docs have the highest burn out rate. Our two specialties get all the “we don’t want to deal with this patient anymore.” Kids don’t go into EM and if you go into IM make sure to subspecialize

My mentor is an IM admin. I think this is a bigger problem in training hospitals where surgery takes primary at all. In systems where medicine is properly staffed and compensated to do this everyone is happier.

Hopefully surgery won't mind if medicine's notes have no updates to the assessment or explanation for sometimes contradictory recs. If they do mind, I hope that they're ok getting yelled at for not implementing the recs perfectly (after being unable to reach medicine for clarification for hours) and that they're fine with the just the explanation of "this is how my attending always does it" in response to questions about the plan

Its like with psych. As soon as patients have any major pychiatric deseases like schizopherina, they are seen as psych patient even though they are hospitalized for something else.
