I have watched it 10 times and for me he is in reality My points 1- he can't see his children face in dream 2- wedding ring 3- the parts in which Michel Caine appear are real confirm by actor himself in interview where he said he asked Nolan which scenes are real and Nolan said scenes in which he is present And in final scene Michael Caine is present

1st point - He couldn't see it until he got rid of his dreams.....remember in Limbo when Mal wanted to show their faces to Cobb and he just wouldn't look. 2nd point - Yes, this is THIS point is very valid as he never wore the ring in reality and only wore it in his dreams 3rd - Oh I was not aware of this but indeed his presence in the scene did edge us slightly more into thinking that it was indeed reality despite the final suspense of the totem because at the end it just wouldnt matter to Cobb if its real or not as long as he saw his children.
