Roger feels like how I felt about him when I first started the anime. He actually just looked like an average pirate and nothing about him looked special when he was introduced even though he was known as the "pirate king"

It doesn’t have much to do with paying attention, if you look back at the execution scene in the anime and manga, roger doesn’t even look that sick. He has perfect teeth, and his hair/facial hair look fine. He especially looks fine in the wano animation remake of the execution. It’s perfectly understandable for more casual one piece fans to not think about his sickness in that scene
