Nevermind. You have to hold Crouch while climbing to manage it.

It's been a long time since I completed this game, so I can't really help you with these questions anymore. I reccommend asking in the steam discussion forum.

Just use noclip to get through that door, save once you're on the other side, quit to desktop and then load that save to continue from inside the door. I don't see any other way to proceed since you flushed the card lol

Make sure the console is enabled (-console in the steam launch settings) and once you're in the game type "ent_fire !picker unlock" (without quotation marks) WHILE looking at the door you want to unlock.
