Intra and extra oral exam should be done @ least annually either by your dentist or hygienist

many dental offices don't, although they should. I'm in Dental hygiene school right now and we do it on every patient. but until I went to school I had never gotten and extra or intraoral exam either. Oral cancer is one of the most underdiagnosed cancers
