I'm a retired old nurse (class of 1977!) and it's been a long time since I've worked in the field. But in my day I have probably, in the process of doing an IM, aspirated blood 3 or 4 times and then had to stop and go get a new syringe and try again. That was the procedure back in that day. So even though in all those years it was rare that I aspirated blood, I wonder what would have happened when I injected the medication into a little blood vessel inadvertently? The dorsogluteal was one of my favorite sites to use, so it's hard for me to see that one go away. The ventrogluteal is so much more awkward to reach, I always thought. So, this is very interesting for me to see, and I'm curious but other Old-Timers think. Thank you for this video.

I'm old school and I was taught to aspirate, & I still do. I am not buying that it is more dangerous to aspirate. That makes no sense to me. I have hit a blood vessel before & aspirated blood. It's kinda rare, but it happens and I wonder if some vaccine injuries weren't due to injecting into a vein. Just a thought... 🤷

I also asperate I just took my shot in my leg and aspirated then started pushing and got the taste in my mouth pulled back again and blood shot in so I pulled out and got a shit ton of blood leak out. I always aspirate if not and you inject into a vein it won't kill you but it will suck. Chest gets tight and makes you want to cough

I agree aspiration should be done but now isn't even taught and is said to be not needed which I hate. I know in school we were taught and they aspirated blood on me. The teacher instructed the student to stop and pull the needle out and to use the other arm.

We do not aspirate anymore when it pertains to the deltoid, however it is still required to aspirate for IM injections in large muscles, this due to the large vessels found in those areas. IM Injections on the vastus lateralis needs to be administered in the lateral side and not in the middle of the muscle thigh as said in the video, the femoral artery runs pretty much in the middle of the thigh. This is of absolute importance especially when administering Epinephrine IM with an Epi pen as this will not allow to aspirate, An accidental release of epinephrine into a vessel would cause a cardiac event.

So the chance of inadvertent intravenous injection is so much lower for the deltoid that it's not worth taking a second to aspirate? Can a case be made for aspiration, as is practice in Germany and Denmark? What's the rush exactly?

@John Harrold I actually agree with you on aspirating even when the injection is meant to go on the deltoid area. It only takes 2 seconds and could prevent complications. I was focusing on the vastus lateralis because the video demonstration is inaccurate and possibly dangerous.

@@baba-sm1fm actually, you’ll need to aspirate for at least 10 seconds for it to be accurate. A common mistake which could give inaccurate information about whether or not you hit a blood vessel, according to literature, is not aspirating for long enough. So if you are going to aspirate, take your time

Kenalog has done wonders for my bilaterial sub acromial impingement. I've had terrible numbness in my arms for quite some time and can't sleep. From what I've researched, it's probably one or a mix of, radiculopathy, thoracic outlet syndrome, cubital and carpel tunnel. Yet, a posterior injection under my acromian this past Monday has completely removed this numbness/pain from my arms. Any guess as to why? Or any way you can point me in the right direction to research this further? Was going to make an appointment with a neurologist for nerve conduction and EMG, just want to make sense of it before hand.

Hi. In choosing a site, the patient is right handed but her left arm has a tattoo that covers the area quite extensively where the injection goes. Does it matter whether you inject into a tattoo or choose the other arm (which will be her dominant arm though ). ? Thx

Hip only do not do it in your thigh or shoulder or any other muscle that you use often. I did my thigh and 4 days out and it still feels like someone hit me with a bat on my leg. I usually get it in my hip butt area and don't feel a thing but this time i tried my thigh. Very bad decision.

Weird, at my hospital, purple/pink means 20G and green means 18G. Blue is 22G

she did a z tract by pulling the skin on both ends, u should not stretch the skin while injecting, once the needle is in then take it off., which she did right but are u not suppose to leave the needle for another 10 seconds after administering the medication?
