By the way I failed to mention that on my Dad's house that was built in the late 1950' and my Dad and Mom bought the house in 1960 had a form of the Gutex insulation on the 2x4 walls with no other insulation. Out of curiosity when looking up the fibrous wood insulation I came up with your website.

The Gutex company has been making wood fiber insulation for over 80 years, and there have been others like it in the past. Natural insulation was the only insulation prior to the chemical era.

You face tape the bottoms of your window assembly? What happens if the window joints get small amounts of moisture seepage?

That's right. Because the materials we're using have such a high water resistance, it's unlikely, but in the case of install error, having materials with such a high vapor permanence ensures that anything that gets in will dry out.

Tape the OSB of the SIP on interior with TESCON VANA. Exterior, either ADHERO or MENTO with TESCON VANA

Right from the beginning I’m going to have to disagree with building an airtight addition on an existing home unless you’re able to talk the homeowner into installing an ERV. I’ve already seen the problems this causes in the form of condensation on windows and mold growth because someone has built an airtight master bedroom/bathroom addition and no ventilation besides a bath fan.

I think you’ve identified a problem without an accurate assessment of the root cause. Mold growth and condensation wont usually be resolved with an ERV (unless perhaps you’re in Arizona). High dew points require a dehumidifier, not more ventilation alone.
