Publishers use SSP's, DSP's are used by the advertisers, the exchanges can be used by dsp, ssp's and can be used directly by the advertiser. Many a times an advertiser goes through an ad agency.

In the video (which was absolutely fantastic), Nick describes what is called a 'second-price' auction, where the second-highest bid plus one cent is the amount actually paid by the winner of the auction. Second-price auctions are also termed Vickrey auctions (en.wikipedia.org/wiki/Vickrey_auction). Why would a Vickrey auction be used, when the publisher doesn't get the highest bid price? Because the dominant bid strategy for a Vickrey auction can be mathematically proven to be bidding the amount you truly believe the item (in this case, an ad impression) to be worth - rather than guessing what the competition will bid, and driving up the overall cost of the impression. While driving up the cost of the impression might generate additional revenue for the publisher in the short term, it will increase overall cost of advertising and, in the long term, reduce demand. ClearCode wrote an excellent blog post (clearcode.cc/blog/first-price-second-price-auction/) which outlines the differences between second-price (Vickrey) auctions and first-price auctions. Now, with all that being said there is now an industry shift toward first-price auctions due in large part to a perceived lack of transparency in the second-price auction process.
