I'm currently in a perio residency, and your videos have been so helpful for understanding the practical/clinical side of implants. Thank you so much!!!!!

Thank you for your kind words. It warms my heart to see the next generation benefiting from the lessons learned by the past generations. If you have colleagues who you believe would also benefit from these insights, please share them with them. Together, we can collectively contribute to improving our industry as a whole.

How to learn from you

I was getting a guided front tooth implant by my colleague in my mentor who is an experienced periodontist and he put it parallel and exactly where it should be so the socshield worked. I tried to duplicate it with simple ext and just to learn how it works It’s hard to make it to work It only worked one time an upper molar, but it took me almost half an hour to leave those two B roots walls . It worked and the patient was happy because we kept her Born bone for free fur future implants or Bridge I won’t do it again.

What would be time line in case if immediate Implant placement at the time of extraction given all protocols followed for an final restoration? Thanks

Great question. I suggest waiting for 3 months and starting the restorative process. The restorative process can vary from a couple of hours to weeks, depending on the aesthetic and functional demands of the patient. For a milled monolith zirconia final prosthetic, it’s usually completed in a few days. Some studies have measured ISQ at the time of placement and attempted to use that to shorten healing time. For instance, with high immediate ISQ values, they may start the prosthetic work up at 7 weeks. While this might be a valid treatment method, I find that it increases the logistical effort required to monitor each individual case, as each one has a different healing time. To simplify the process, I suggest setting an “always” healing period and adhering to it for everyone.

Let me guess. Socket shield is: 1.Good for saving the vestibular tissue level 2.Bad for no implant integration on that side

Yes, it is beneficial for maintaining labial tissues. I wouldn’t call the implant integration process bad. It’s technically challenging, and complications can arise if the process isn’t executed perfectly. This could lead to an upset patient with a complication in the esthetic zone. No one wants to deal with that. That’s why I propose skipping this technique for the “maintain not regain” method. .

Yes. I think it is not worth of doing it. More risk without increased benefits
