Q1: Pyrazinamide, ethambutaol, rifampicin, isoniazid and Streptomycin. Q2: Isoniazid is a Prodrug which is activated to isonicotinic acid by katG enzyme present on the surface of M.tuberculosis. Isonicotinic acid inhibits baterial cell wall mycolic acid, that's why making M.tuberulosis susceptible to reactive oxygen radicals. Q3: Isoniazid extensively metabolized to inactive metabolites. The major metabolite is N-Acetyl isoniazid and the enzyme is cytosolic N-acetyl transferase.

Q-2 Isonicotinic acid activated form of isoniazid inhibit the bacterial cell wall mycolic acid. Ans - 2 The main pathways of INH metabolism include: Acetylation by N-acetyltransferases to form acetyl-INH (AcINH), and (2) Hydrolysis by amidases, producing isonicotinic acid (INA) and Hydrazine (Hz). AcINH can also be hydrolyzed to form isonicotinic acid (INA) and acetylhydrazine (AcHz).

1: First line tb drugs: isoniazid, streptomycin, rifampicin, ethambutol, pyranzinamide Second line : amikacin, para amino salicylic acid, ethionamide, viomycin, ciproflocaxin 2: Isoniazid is an inactive drug it is activated by KatG enzyme on surface of M.tb into isonicotinic acid that inhibits the bactetial cell wall containing mycolic acids making M.tb susceptible to reactive oxygen radicals. 3: isoniazid convert in major metabolite N-acetyl isoniazid by enzyme acetyl tranferase. Acetyl isoniazid further convert into two products :(a) isonicotinic acid which forms conjugate with glycine in urine (B) acetyl hydrazine which either gets aceylated into inactive diacetyl hydrazine or convert into reactive metabolites by hepatic microsomal enzymes i.e hydrazines which induce hepatotoxicity .
