Still Not a Player went platinum, he had multiple singles chart in the top 100, Capital Punishment was nominated for a Grammy - he had plenty of mainstream success. He had a massive heart attack and passed just 4 years after his first album. It had nothing to do with audio quality, he just passed before he was really able to become a mainstream household name

I interpreted him saying "Pac off the top" as in off the top of his head/right off the bat because he's arguably the most iconic rapper and he came to mind first. But you could also take that statement as his favorite and at the top off his list I suppose. Regardless of what he meant Eminem would still be in his top 3 current living rappers.
