If Jenny’s Grandma was the one who was sick then why did Jenny die later in the season?

Its shown that Jenny dies of a fatal heart condition, which she hid from her friends.

@@purplebutterfly5366 I guess it was after she fainted at Gale's wedding. Everyone goes to the hospital, but Grams sends them all home, assuring them everything’s okay, but she couldn't convince Jack, who sticks around to discover that Jen has a heart condition, called pulmonary congestion, and it’s fatal.
