Trolley Driver: I continue on my scheduled route: my expectation is that the workers have to play an active role in their work environment. The bystander / tied down versions... ? Bystander version more closely resembles transplant doctor scenario than trolley driver?

Really great articles by the authors (I have not read) and explanation by you. I was talking to a friend about health care and he mentioned the waste of money that some programs are. The surgeon reference made me think of it, but do these philosophical questions become relevant when we decide what we should fund (and the amount of allowable waste) in healthcare or causes we should donate to?
