IVC/Portal vein thrombosis can cause b/l pedal edema. I suppose that was an initial clue, that the heart was fine, but IVC wasnt, given that there was no JVD. From there, you can think CT liver/kidney for RCC. Hence, the diagnosis was HCC, but part of it was Budd Chiari Syndrome.
